Nasal Turbinate Radiofrequency in İstanbul, Turkey

There is no definite consensus on the most effective treatment for hypertrophy of the inferior turbinates. However, nasal turbinate radiofrequency uses radiofrequency to create lesions within the submucosal tissue, reducing turbinate volume with minimal impact on surrounding tissues (irt can cause limited mucosal damage).  Tubinates should be accepted as an organ and their anatomical structures should be preserved as much as possible. When there is an unnecessary loss of abnormal volume or when turbinate resection is performed, an increase in nasal air flow velocity (nasal hyperventilation) and empty nose syndrome may occur in the case of complete surgical excision. Respect for tissue is very important for preserving nasal functions.

Hypertrophy of The Inferior Turbinate


Mucosal hypertrophy of the inferior turbinate is the most common cause for nasal obstruction and second cause for nasal obstruction is septum deviation. Turbinate hypertrophy can be treated as medical or surgical methods.

Symptoms of Turbinate Hypertrophy


Turbinates are tissues located in the nasal cavity, both on the outside, which are involved in the humidification, purification and heating of the air. There is a continuous cleaning process from front to back. Nasal meats may grow gradually depending on conditions such as allergies, drug use, infections and environmental conditions.




Allergic Rhinitis and Turbinate Hypertrophy

Due to turbinate hypertrophy, air resistance entering the nose gradually increases and symptoms such as open mouth sleep, nasal congestion, dry mouth at night, night sweats, bad breath may occur. The severity of these symptoms depends on the size of the turbinates of the nose.

  • immunotherapy
  • antihistamines
  • intranasal corticosteroid sprays
  • decongestants
Surgical treatment for hypertrophy of the turbinate:
  • corticosteroid turbinate injections
  • cryosurgery
  • electrocautery
  • turbinate out-fracture
  • microdebrider-assisted turbinoplasty
  • excision of turbinate
  • submucous resection

In which cases is turbinates radiofrequncy done?


Turbinates radiofrequency can be performed when turbinate hypertrophy is caused by the occurrence of complaints. In case of "Rhinitis Medicamentosa" due to long-term use of nasal openers, CPAP (Continuous Positive Airway Pressure) treatment can also be performed in order to increase patient tolerance. In patients with turbinate hypertrophy, if there are persistent complaints such as yor early fatigue while walking, difficulty climbing stairs, waking tired from sleep, dry throat, frequent pharyngitis infection ", minimal traumatic turbinate reduction procedures are performed for the treatment of nasal congestion.

How is turbinates radiofrequency performed?


Turbinates can be performed under radiofrequency, local or general anesthesia. Modern radiofrequency instruments (Celon, ArthroCare, Gyrus) that measure tissue resistance and cut the current without giving much energy to the tissue can be performed very easily and without complications.

Turbinates radiofrequency processing time


Turbinates radiofrequency process usually takes a few minutes. When other nasal surgeries (septoplasty, nose type plasty, endoscopic sinus surgery ... etc.) are performed, the operation time is prolonged.

Turbinate radiofrequency without packing


When turbinates radiofrequency is performed with tissue resistance measuring devices and mimimal mucosal trauma, the procedure is usually terminated without buffering. Postoperative tamponade may be more appropriate in patients with hypertension, nosebleeds and vascular disease.

After turbinates radiofrequency, bleeding from the anterior and nasal regions may occur as dripping or leaking.

In order to decrease the blood pressure in the head area, it is appropriate to place the patient in a lying position by ensuring that the patient's head is above the body.

The room temperature of the patient and the temperature of the water applied to the face while taking a shower should not be too high. As the width of the nasal vessels will increase in the hot environment, the amount of bleeding may increase. Bleeding in the form of this leak will decrease within days. Exercise, sportive activities and professional duties that increase blood pressure after surgery are inconvenient for the first 10 days.

The most important factor that accelerates recovery after surgery is washing the nose with ocean water or other salt water. Ocean waters provide the removal of the clots, increase the nutrition of mucosal cells and the removal of microorganisms. The feeling of dryness and crusting in the nose in the first few weeks after surgery is expressed by most patients. Therefore, intranasal moisturizers such as Rinopantenina Drop, Siccnasal Ointment may also be useful.

Radiofrequency (RF) technology is used in medicine for treatment of many health condition. Radiofrequency devices are classified according to on the number of electrodes used as unipolar devices, bipolar devices, or multipolar devices. Radiofrequency energy induces ion agitation within the tissue, which increases the local temperature and causes a thermal damage in  the deep tissue without damaging the surface mucosa. The healing process secondarily cause to fibrosis, wound contraction and resulted with tissue volume reduction of turbinates.

This procedure can be performed in an outpatient setting under local anesthesia or it can be performed with other nasal surgeries such as septoplasty, polypectomy, rhinopasty as under general anesthesia. Local anesthetics are injected into the turbinate after lidocaine spray application (use of local anesthetic without adrenaline is important for understanding the areas with the highest growth). Submucosal areas of the nasal meats are then reduced in volume with a radiofrequency device. Generally during the process, the shrinkage of the turbinate dimensions can be seen. After the process, shrinkage usually lasts up to 1 year. The total procedure time is about a few minutes under local anesthesia. Bleeding may occur in the first few days after the procedure, and turbinates may swell over the first few days, causing nasal congestion.
